HEALTHY START CASE MANAGER

1. INTRODUCTION

The purpose of this job description is to report the functions, requirements, working conditions, and office responsibilities of the Case Manager. This position reports directly to the Supervisor for Healthy Start.

2. ESSENTIAL FUNCTIONS

The basic functions of the Case Manager are to provide initial outreach via Coordinated Intake & Referral and Healthy Start services during the pre-natal and post-natal care period to pregnant women, These services are also provided to newborns and children up to three years of age, and are held in participating physician’s offices, clients’ homes, or any setting deemed appropriate by the counselor and client. Some essential duties are, not limited by, the following:

All Case Managers:

  • Provide pregnant mothers information on selecting an obstetrical provider, explain the importance of early prenatal care, the purpose of the Healthy Start screening and provide contact information on how to select a Medicaid managed care plan
  • Initiate the Healthy Start Care Coordination record
  • Receive service requests from referrals for pregnant and postpartum mothers, inter-conception and inter-conceptual women, and infants who are residents of St. Lucie County.
  • Conduct assessment and evaluation of clients’ health needs including OB or pediatric provider choice/access, insurance coverage and explanation, mental health needs, and education on basic health, nutrition, and infant care
  • Secure and protect client confidentiality at all times
  • Assist in overall OB or pediatric service delivery issues and basic educational needs for pregnancy-related health and/or infant care during home visitation
  • Monitor client progress and appointment compliance on choosing and accessing OB or pediatric services for the infant
  • Conduct home visits upon referrals for pregnant women, newborn infants, and postpartum mothers; address clinical and social health issues
  • Conduct follow-up home visits for clients, when necessary, after initial home visit determines the need for further services
  • Conduct hospital visits of newborns and mothers (when possible) to facilitate future service encounters and needs
  • Refer and/or connect clients to other community programs outside the Healthy Start system, when needed and/or feasible, such as Medicaid, WIC, and Family Planning Medicaid
  • Provide referrals to Healthy Families of St. Lucie County as needed; route client to Coordinated Intake & Referral with client’s agreement
  • Complete service reports and evaluation components; communicate with other service providers at a minimum of once per month on the clients’ needs and concerns
  • Inter-conceptual education to be provided to the woman throughout the period of time she is open for services
  • Present, participate, and/or support educational classes as needed
  • Provide guidance and direction with teen cases and health education classes

Spanish Speaking Case Managers:

  • Work with other providers to facilitate communication with the client that only speaks and reads in Spanish
  • Provide translation services of written material for the client as needed

3. OTHER FUNCTIONS

  • Support with projects as needed, including but not limited to health fairs and events
  • Completes any additional assignments as requested by management
  • Attend staff meetings, coalition meetings, and trainings as required

4. MINIMUM JOB REQUIREMENTSEDUCATION / EXPERIENCE

Bachelor’s Degree in Social Sciences, Social Work, or another health related field such as nursing, health education, health planning, or health care administration is preferred. May substitute two years’ of college with a minimum of three years’ of public health or community development experience.

SKILLS/KNOWLEDGE

  • Exceptional scheduling and/or prioritizing skills with strong data entry proficiency
  • Proven knowledge of prenatal and postnatal screenings
  • Ability to handle multi-phone lines effectively
  • Proven ability managing multiple priorities; strong organization, detail and process management orientation
  • Working knowledge of office machines and equipment, e.g. fax machines and photocopiers
  • Strong proficiency with computers, data base entry and with full Microsoft Office suite, email, and internet
  • Excellent written and verbal communication skills; above average interpersonal skills
  • Ability to read, analyze, and interpret data and information and apply appropriate judgment
  • Passion for providing extraordinary customer service and work with people at all levels
  • Demonstrated ability to work independently with supervision: ability to make decisions independently
  • Honesty and integrity are critical to this organization
  • Exercise good judgment and maintain confidentiality of critical and sensitive information, records, and reports.
  • Understand, interpret, and apply general administrative and departmental policies and procedures.
  • Ability to work well independently and in a team environment
  • Ability to balance multiple assignments and adjust direction and priorities within fast paced and multi-faceted work environment

Travel

Extensive travel is normal for this position to the client’s home, physician offices, hospitals, etc. Must have a valid Florida driver’s license.

5. Working conditions

Fast paced work environment; flexibility to working schedules may be required due to changing business demands. General office conditions is the standard work environment (standing, sitting, and walking). An equal opportunity employer and a smoke, drug and alcohol free workplace.
