Healthy Start Team Member
Looking for dynamic team member with a heart to serve mothers, babies and families in Indian River County.
As a member of both the Cleveland Clinic team and Healthy Start, the position offers the opportunity to be an impactful member of the county’s healthcare community. Through the leadership of Healthy Start’s well-established programs that have been embraced and supported by the community, team members will interface with patients, families, physicians, staff and external agencies.
BASIC FUNCTION:
This position is for a highly responsible professional who provides education, case management, care coordination and enhanced services to pregnant women and children to age three.
SUPERVISORY ACCOUNTABILITY: None
NATURE AND SCOPE:
This position interacts with patients and families, Healthy Start staff, nursing staff, and community healthcare resources. The position is responsible for home visitation, office, and hospital based services.
PRINCIPAL ACCOUNTABILITIES:
Supports the mission, vision and values of the hospital.
Responsible for working in a safe and protective manner at all times, keeping in mind that safety and environmental hazards are the responsibility of all employees for themselves, other staff members and patients.
Create and maintain a positive image of the Indian River County Healthy Start Coalition, Cleveland Clinic Indian River Hospital, and the Healthy Start programs.
Provide case management and ongoing care coordination according to the risk appropriate criteria for all Healthy Start participants in accordance with the “Healthy Start Standards and Guidelines” and local Indian River County Healthy Start coalition criteria per the community needs assessment.
MINIMUM REQUIREMENTS:
Graduation from an accredited college or university with a Bachelor’s Degree in health planning, public health, health education, social services, or related field preferred, and/or two years of relevant work experience.
Two (2) years of relevant experience.
Computer skills including web browsing, e-mail, internet, and word-processing.
Reliable transportation.

Healthy Start uses an empowerment-based home visitation model to support and educate parents to be the best they can be.
Healthy Start’s Programs: These programs are highly integrated into the maternal and child health system and are pivotal in the support of families and the prevention of infant mortality.
Babies and Beyond program (serving 95% of all moms in county with prenatal education, childbirth education, lactation support, newborn home visitation),
Health Education Program (Health Education-Prenatal)
Maternity Navigation/ Coordinated Intake & Referral program (connections for all moms)
Community Doula program (providing doula care for 200 moms a year)
Parents as Teachers (Parenting education and empowerment)
