Duties and Responsibilities:
1. Hires, trains, supervises, motivates, schedules, and evaluates care giving staff to ensure the provision of high-quality services to Residents in a manner that is consistent with the philosophy of assisted living and with the regulations set forth by the Executive Office of Elder Affairs.
2. Works together with full time Assistant Health and Wellness Director to oversee operation of the entire wellness department and together provide overall support to the Nursing and C.N.A staff. Rotation of weekend on call with Assistant Director is required.
3. Oversees orientation of new staff. Develops in-service training and provides ongoing training of staff consistent with the regulations set forth by The Executive Office of Elder Affairs.
4. Evaluates the skills and performance of care giving staff at least twice a yearly. Provides on-going supervision to care giver staff.
5. Provides 20 hours of personal care training to those employees providing personal care who have not completed a state recognized training program for STNA.
6. Plans and delegates work assignments assuring effective scheduling on a 24-hour basis.
7. Completes pre-occupancy screenings/assessments and develops the Resident's Individualized Service Plan in conjunction with the Resident, his/her family, the care giving staff and other necessary individuals, prior to entry into Keystone Place at Buzzards Bay.
8. Assists in the coordination of the move-ins of new Residents. Orients new Residents ensuring satisfactory integration into the Community. Provides Introductory Visits by reviewing each Resident's Service Plan with all Resident Care Associates prior to or within 48 hours after they are to provide Personal Care Services to that Resident.
9. Supervises medication administration in compliance with regulations set forth by the Massachusetts Executive Office of Elder Affairs. Follows through on verbal medication change orders received from the physician and documents in wellness files as appropriate.
10. Monitors and updates Resident Service Plans as necessary, records information to determine the services to be provided to the Residents, and directs caregivers to implement needed changes. Schedules and leads service plan reviews with the Residents and their Responsible Parties, at least biannually or at the time of a significant change in the Resident's health status.
11. Facilitates and encourages family involvement with Residents.
12. Monitors and maintains wellness records reflecting the Residents' physical and mental conditions. Maintains communication with the Resident and/or his/her Responsible Party and physician when needed to ensure that all medical needs of the Resident are met.
13. Coordinates and oversees temporary and/or permanent Resident transfers or move-outs to other facilities.
14. Investigates Resident and employee incidents, identifies safety hazards and institutes corrective action in a timely manner. Shares in the responsibility for providing and maintaining an environment that is conducive to the safety and the physical and emotional well-being of Residents and personnel.
15. Documents Resident incidents appropriately, and follows up as necessary.
16. Supports and participates in the Resident-centered activity program.
17. Demonstrates a true appreciation of the heritage, values and wisdom of the Residents and an understanding of the aging process.
18. Demonstrates the ability to function as a team member in sharing responsibilities for the administration of Resident services in cooperation with all members of the team.
19. Promotes and facilitates interactive problem solving with staff.
20. Performs other duties as assigned by the Executive Director.
Qualification/Specifications:
Education/Experience: Must be a licensed nurse (LPN/RN). Must possess a high degree of interpersonal relations skills and the capability of relating to a variety of people and personalities. Considerable initiative, judgement and leadership skills are also imperative. Must have the demonstrated ability to hire, direct, and manage personnel and possess strong organizational and time-management skills.
Previous experience in assisted living preferred.
Complexity: Ability to plan and prioritize work duties. Must be able to prioritize Residents' needs. Must be self-directed and industrious.
Supervision: Under the supervision of the Executive Director. Must be able to follow through without close supervision.
Other Contacts: Constant contact with Resident, potential Residents, their families, and various health care providers. Must be courteous with all contacts.
Confidentiality: Will be exposed to highly confidential information, the disclosure of which would be contrary to the best interest of the Residents, the Community and professional ethics.
Physical Demands: Good physical ability to perform high levels of physical activity for extended periods of time. Must be able to lift/push up to 50 pounds multiple times during a shift. Physical dexterity required in lifting, pulling, pushing, carrying, stretching, moving, bending and stooping. May sit or stand for extended periods of time.
Working Conditions: Clean, well-lighted household environment. Will be constant activity and frequent interruptions. Exposure to body fluids, infection, odors, and of Residents. Exposure to chemical/cleaning solutions.
Safety: Must utilize Universal Precautions when assisting Residents and/or handling any body fluids.
