What You’ll Do:
- Monitor patient health status and follow up on care directed by providers.
- Coordinate comprehensive care plans tailored to chronic condition management.
- Build lasting relationships with patients and families to ensure ongoing health support.
Requirements:
- Active Idaho LPN license
- Compassionate communicator
- Valid driver’s license and reliable transportation
