The Key Program, Inc. is seeking a dedicated Service Coordinator (Case Manager) to support youth and families served through the Lead Agency Program in the Greater Springfield Area. This role is embedded directly within the Department of Children and Families (DCF) regional office and plays a vital part in coordinating services, supporting placement decisions, and ensuring high‑quality care across providers.
Wage: Competitive salary wage: $56,549-$58,709 Additional compensation is provided for bilingual skills and our generous employee referral bonus program.

Qualifications:
One of the following:
- Bachelor’s degree in a related human service or clinical field
- Minimum of 1 year case management or direct care experience
And
- Must be 21 years of age or older
- Valid state driver’s license and legally registered and insured car for work use
Responsibilities:
As the Service Coordinator, you will ensure that youth and families receive the right services at the right time. Responsibilities include:
- Coordinating and managing services between families, DCF, and provider agencies to ensure appropriate placement and support
- Provide referral information to potential service providers, schedule utilization reviews and ensure that appropriate services are provided to DCF children and families
- Facilitating treatment plan reviews, diagnostic conferences, and discharge planning meetings
- Monitoring service implementation and ensuring consistent case coordination
- Collaborating with providers to ensure accurate and timely documentation, billing, and data entry
