Salary: $29.41/hour - $47.06/hourDepartment Summary: Kent County Local Management Board serves as one of 24 LMBs in Maryland. KCLMB coordinates services and investments for children, youth, and families. This includes planning activities with the agencies and organizations that serve children, funding programs that improve services and fill gaps, and raising awareness among partners, families, and providers about relevant topics like trauma-informed approaches, demographic disparities, and civic engagement. Each LMB houses the Local Care Team to provide this coordination between local agencies and families directly.Position: The Systems of Care Coordinator ensures a coordinated system for Local Care Team case referrals and tracking, maintains a comprehensive resource database, and ensures follow-up services as necessary to help ensure referred youth receive comprehensive support services. They are responsible for facilitating a collaborative approach to services and ensuring parent and youth involvement in Local Care Team meetings.The ideal candidate for this opportunity is familiar with local and state resources; has experience with case management, special education, human services, strategic planning, negotiation, team building, and networking; demonstrates cultural humility in responding appropriately and effectively to individuals with a range of needs, competencies, and backgrounds; is familiar with statistical, results tracking, and case management programs; has good listening, communication, and mediation skills; and is comfortable reading, analyzing, interpreting, and producing formal documents.This is an excellent opportunity to join a cohesive, passionate team and work with a dynamic interagency workgroup committed to improving the lives of children, youth, and families.Essential FunctionsFunctions listed are intended only as illustrations of the various types of work performed. The omission of specific duties does not exclude them from the position if the work is similar, related, or a logical assignment to the position.Manage the Local Care Team Kent Countys multi-agency team to support families with children with intensive needs. The team meets with families every two weeks to create action plans and ensure the effective coordination of services.Facilitate and mediate Local Care Team meetings to promote constructive dialogue, balanced participation, and family-centered problem-solving.Coordinate Kent Countys Systems of Care through advocacy, education, service expansion, and coordination. Strengthen interagency collaboration to prevent duplication or gaps in services.Manage the Brighter Day Fund the Local Care Teams fund for miscellaneous needs, through grant acquisition and implementation.Maintain, analyze, and produce written reports from various data systems, and develop corresponding policies and procedures.Provide staff support to local and statewide committees.Develop and present training modules and Local Care Team outreach to groups and stakeholders.Provide technical assistance to vendors, partner agencies, and community groups.Maintain and publicize a directory of community resources.Manage program administrative tasks like drafting contracts and filing invoices.Communication and outreach to strengthen the departments visibility in the community.Performs other duties as required.Supervision: The position reports to the Director, Kent County Local Management Board. The position supervises contractual staff and grantees.Required Knowledge, Skills, and AbilitiesThe employee is expected to perform or possess the following:Communication Skills: Ability to build and maintain effective relationships with Director and Board members, Local Care Team partners, vendors, award recipients, and community groups. Anticipates emerging needs and adapts accordingly, building collaboration and buy-in for new initiatives.Technical Skills: Effectively utilizes standard database software to write queries, develop relational databases, and run reports. Ability to use the Internet and social media resources, e-mail, spreadsheet, and office software. General knowledge of PC equipment.Education and Experience: Bachelors degree, with a preference for a Masters Degree in a related field, and five to seven years experience.A clinical, social work, or special education background. Or equivalent technical training, education, or experience with child placement systems.Valid Maryland Drivers License and an acceptable motor vehicle record.Knowledge of standard office and database software, plus knowledge of Internet and social media functions.Physical and Environmental Conditions:Work is conducted in a normal office setting, which provides comfortable lighting, temperature, and air conditions.
