Department/Location: Care Management adult mental health

Compensation: $17.24 - $22.86 hourly. The final job offer is based on several factors (internal equity, skills, education, experience, and credentials).

The Care Management (peer) Support Specialist provides support to the Care Management Program Supervisor, supporting enrollees with complex behavioral and chronic physical health conditions, in collaboration with one or more health plans and providers in Network180’s system of care.

 

This position is 40 hours a week, Monday through Friday, 8:00 am to 5:00 pm. There are no weekends, on call, or holiday requirements for this position. This assignment is in person at 790 Fuller Ave Grand Rapids, Michigan 49503 and within the community.

 

MINIMUM QUALIFICATIONS: 

  • High School diploma or general education degree (GED). 
  • State of Michigan Certified Peer Support Specialist or be eligible to become certified by the State as a Peer Support Specialist, which requires an individual to have:
    1. Primary diagnosis of a mental health condition
    2. Have a strong personal knowledge of what it is like to have first-hand lived experience with a mental health condition that has caused a substantial life disruption
    3. Has been a recipient of mental health treatment and/or services for at least one year, with a substantial life disruption due to their mental health condition 
    4. Have personal experience in navigating complex mental health treatment services, and/or 
  • State of Michigan Certified Peer Recovery Coach or be eligible to become certified by the State as a Peer Recovery Coach, which requires an individual to have:
    1. Long-term, stable recovery from alcohol and other drugs 
    2. Identifies with another individual based on their shared life experiences to promote recovery by removing barriers and obstacles to recovery
    3. Has been a recipient of mental health and/or substance abuse services for a least one year, with a substantial life disruption due to their substance abuse condition
    4. Have personal experience with navigating complex substance abuse treatment services. 
  • Possession of a valid driver’s license and access to reliable transportation for job related use
  • Recovery oriented and experience with emotional or other emergency/crisis planning. 
    • Computer skills, including word processing, spreadsheets, email, and data entry skills.
    • Excellent communication skills, both verbal (face-to-face or via phone) and written.
    • Demonstrated experience utilizing effective problem-solving skills.
    • Work effectively in a multicultural environment and skills and capabilities of bridging language/cultural barriers to Network180 services within diverse communities

 

PREFERRED QUALIFICATIONS:

  • Experience working with complex clients who experience both psychiatric and physical co-morbidities.
  • Working knowledge of health care integration concepts.
  • Knowledge of basic health and wellness strategies and/or lived experience with the management of a chronic physical health condition.
