Schedule:
- Monday to Friday availability
- 8:00 AM – 4:00 PM
- Flexible: Choose 2–3 days per week
- Total hours: 16–24 hours weekly
Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
- Monitor and observe patients’ health status, recording vital signs and medical observations accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Administer medications following prescribed schedules while maintaining strict adherence to HIPAA regulations and medical documentation standards
- Support clients with specialized care needs such as Alzheimer’s, dementia, memory care, hospice, or developmental disabilities
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment
- Assist with medical procedures such as catheter care, wound care, or other nursing tasks under supervision
- Document all patient interactions thoroughly and accurately in compliance with healthcare regulations and agency policies
Experience
- Prior experience in home health care or assisted living environments is highly preferred
- Demonstrated ability to work with individuals with disabilities or complex medical needs such as dementia or hospice patients
- Familiarity with EMR/EHR systems and medical terminology enhances efficiency and accuracy in documentation
- Knowledge of CPR certification and skills in vital signs measurement ensures readiness for emergency situations
- Previous caregiving roles involving medication administration, catheter care, or hospice support are valued assets
- Strong communication skills in both Spanish and English are essential for effective client interaction and team collaboration
