Position Summary & Responsibilities

The position (K0230022)
The Kansas Department of Health and Environment (KDHE) Health Facility Surveyor (HFS) conducts initial, recertification, complaint and or follow-up revisit surveys of Medicare/Medicaid certified healthcare facilities for non-long term care provider and supplier types (e.g. includes but not limited to hospitals, home health agencies, hospice agencies, ambulatory surgery centers, rural health clinics, End Stage Renal Disease facilities, etc.). These surveys are conducted to determine compliance with the Centers for Medicare and Medicaid (CMS) conditions of participation or conditions for coverage. The HFS additionally conducts surveys to determine compliance with Kansas state statutes and regulations for non-long term care providers and suppliers requiring Kansas licensure. This position plays a critical role in protecting patient safety, ensuring quality of care, and enforcement of CMS standards and state licensure requirements.
Essential duties and responsibilities of the HFS include but may not be limited to the functions mentioned in the job description. During the survey process the HFS evaluates compliance with applicable federal regulations and state health codes. The HFS observes care delivery, reviews clinical and administrative records and interviews staff, patients, and families; investigates complaints and facility-reported incidents using established protocols; determines non-compliance and documents findings (Statement of Deficiencies) with clear, factual supportive evidence using established protocols (CMS Principals of Documentation) and writing guidelines. The HFS is responsible to ensure documentation is accurate, objective and legally defensible. The HFS works as part of a multidisciplinary team, participates in pre-survey planning, entrance and exit conferences, and debriefings. The HFS may offer guidance to facilities on how to interpret and apply regulations (without acting as a consultant). The HFS may participate in the review and evaluation of Plans of Corrections. The HFS is expected to complete the CMS Surveyor Training Course prescribed and conducted by the State Survey Agency and participate in ongoing education to stay current on changes in CMS guidance, federal laws, and state requirements. The HFS aids in orientating and training of newly assigned HFSs to perform assigned job duties and responsibilities. The HFS may provide testimony and input for the preparation of documents and other materials needed for case hearings and related proceedings. The position requires travel. Travel will occur during all-weather conditions. Some travel will require overnight stays and
survey work during off-hours and possibly weekends.
Job Responsibilities may include but are not limited to the following:
Conducts onsite surveys of acute and continuing care providers and or suppliers to determine compliance with appropriate state licensure requirements and/or federal certification requirements in the Title SVIII (Medicare) and XIX (Medicaid) programs; Completes required survey forms and makes licensure and certification recommendations.Onsite surveys include observation, record review and interviews with facility staff, residents/clients/patients and their family members.Onsite surveys are conducted according to the federal State Operations Manual (SOM)and state statutes or as directed by state directives. Onsite surveys are conducted individually or by a team of surveyors.Onsite surveys are conducted with the individual team member participating in or conducting the entrance and exit conferences.Interacts knowledgeably on state and federal regulations with the health care providers, the public, peers, and supervisors.Maintains effective professional interpersonal communication skills with health care providers, the public, peers, and supervisors. Completes all documentation as required according to established criteria and the federal SOM. Completes and submits documentation within the required time frames. Documents observations, record review and interviews on the required forms. Compiles and documents deficiencies according to the Principles of Documentation.Organizes survey packets, and prepares written reports as directed/assigned.

Qualifications
- Education:
- High School/GED required
- Licensing & Certification:
- Valid Driver's License - Incumbent is required to have and maintain a valid driver's license when operating a state vehicle, a private vehicle, or a rental vehicle for the benefit of the State.
- Licensed registered nurse and experience in a health care facility/service.
- Minimum Qualifications:
- Licensed as a registered nurse and experience in a health care facility/service.
- Knowledge of the principals of public health and medical care administration; interviewing techniques, current methods, and practices in health care facilities; health care facility equipment, construction, and safety; federal Medicare and Medicaid requirements.
- Preferred Qualifications:
- Experience as a Health Facility Surveyor preferred but not required with knowledge of State and Federal survey and certification requirements for Acute and Continuing Care and or with Long Term Care provider types.
- Ability to interact with individuals and establish a rapport, analyze, prepare, present, and interpret reports and other documents related to health care facility survey activities; interpret and apply related statues, regulations, policies and procedures for health care providers and evaluate health care facility practices in areas of care and treatment.
- Graduation from an accredited nursing program.
