What You’ll Do
- Provide clinical pharmacy services including medication regimen reviews and therapy optimization for LTC residents
- Collaborate with physicians, nurses, and facility staff to ensure safe and effective treatment plans
- Ensure compliance with state/federal regulations and LTC pharmacy best practices
- Serve as a subject matter expert for on-site pharmacy operations and medication management
- Manage medication carts, inventory, and changeovers to maintain accuracy and availability
- Investigate and resolve MAR discrepancies and medication issues in real time
- Partner with pharmacy teams to improve workflows, refill processes, and cycle management
- Utilize electronic health records (EHR) and platforms like MediProcity for documentation and communication
- Train and educate facility staff on medication ordering and system processes
- Act as a liaison between pharmacy and facility teams, resolving operational and clinical concerns
- PharmD from an accredited program (required)
- Active Pharmacist license and Controlled Substance license through the state of Michigan (required)
- Long-term care pharmacy experience preferred
- Experience in geriatric pharmacotherapy strongly preferred
- Strong understanding of pharmacy regulations and compliance standards
- Proficiency with pharmacy systems (Framework experience a plus)
- Clinical expertise and strong analytical/problem-solving abilities
- Excellent communication and collaboration skills
- High attention to detail and organizational skills
- Ability to work effectively within a multidisciplinary healthcare team
- Adaptability in a travel-based role
- Travel between pharmacy and client facilities in Saginaw
- Combination of on-site facility work and pharmacy collaboration
