Job Description
- Lead and coordinate the full MDS assessment process.
- Develop, review, and update individualized resident care plans.
- Collaborate with physicians, nurses, and other healthcare staff to ensure accurate and timely assessments.
- Maintain compliance with state and federal guidelines for MDS assessments.
- Monitor documentation for accuracy and regulatory standards.
- Provide education and support to staff regarding the MDS process.
- Contribute to quality improvement initiatives related to resident care.
- Strong knowledge of MDS regulations, documentation standards, and compliance requirements.
- Excellent organizational skills with attention to detail.
- Effective communication and leadership abilities.
- Proficiency with electronic health records and MDS software systems.
