Experience: Previous experience in MDS coordination or a similar role in a healthcare setting.

Kahu Malama Nurses is seeking an MDS Coordinator to join our team. The MDS Coordinator will be responsible for coordinating the completion of the Minimum Data Set (MDS) assessments for all residents in accordance with state and federal regulations. This role involves working closely with healthcare providers to ensure accurate and timely completion of assessments.

Job Description

• Lead and coordinate the full MDS assessment process.
• Develop, review, and update individualized resident care plans.
• Collaborate with physicians, nurses, and other healthcare staff to ensure accurate and timely assessments.
• Maintain compliance with state and federal guidelines for MDS assessments.
• Monitor documentation for accuracy and regulatory standards.
• Provide education and support to staff regarding the MDS process.
• Contribute to quality improvement initiatives related to resident care.
  
  

Position Requirements

• Strong knowledge of MDS regulations, documentation standards, and compliance requirements.
• Excellent organizational skills with attention to detail.
• Effective communication and leadership abilities.
• Proficiency with electronic health records and MDS software systems.
