Kahu Malama Nurses is seeking a Home Health Clinical Manager RN to join a growing and dynamic team!

Job Description

• Oversee and support clinical operations for adult home health patients, ensuring quality care and regulatory compliance
• Conduct and review comprehensive patient assessments, including Admissions, Start of Care (SOC), Recertifications, Resumption of Care (ROC), Routine Visits, Transfers, and Significant Change in Condition evaluations
• Provide case management oversight and coordinate services with patients, families, physicians, and interdisciplinary care teams
• Ensure accurate and timely completion of clinical documentation, including OASIS assessments and care plan updates
• Monitor and support skilled nursing care for a wide range of patient needs
• Pain assessment and management using verbal and nonverbal pain scales
• Oxygen administration and oxygen safety education
• Post-surgical care, including post-amputation and post-total joint replacement recovery
• Ensure adherence to infection control protocols and clinical best practices
• Provide clinical guidance, problem-solving support, and mentorship to field staff as needed
  
  

Position Requirements

• Active Registered Nurse (RN) license in good standing
• Required experience in Adult Home Health
• Strong experience with admission assessments, case management, and OASIS documentation
• Demonstrated competency in complex skilled nursing procedures and patient care management
• Excellent clinical judgment, organizational skills, and attention to detail
• Ability to work independently and manage multiple priorities in a fast-paced environment
• Travel experience required
• Valid driver’s license and reliable transportation
