Just Family Center, a part of the Sevita family, provides home and community-based services including day programs for elders with complex medical or rehabilitative needs, attendant care and case management. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.
Licensed Practical Nurse (LPN)
Location: Burkesville, KY
Schedule: Full-Time, Monday–Friday (Day Shift)

Position Overview
- Monitor the health conditions of individuals served and ensure quality nursing services are provided
- Administer medications, maintain accurate documentation, and oversee staff responsible for medication administration
- Provide prescribed medical treatments according to physician orders and deliver personal care services as needed
- Conduct health assessments and implement healthcare services within each Individual Service Plan (ISP)
- Monitor and respond to changes in individuals' health conditions
- Provide staff training on infection control, universal precautions, medication administration, and other healthcare topics
- Serve as a liaison with physicians, healthcare providers, and community agencies to coordinate quality care
- Educate individuals served on topics including self-medication, prescribed treatments, smoking cessation, and health promotion
REQUIREMENTS – PLEASE READ CAREFULLY ️
To be considered for this position, all requirements below must be met:
- Current Licensed Practical Nurse (LPN) license in the state of residence
- Associate's Degree in Nursing or related field, or state nursing certificate
- Minimum one (1) year of nursing experience working with the population served preferred
- Valid driver's license, current vehicle registration, and proof of auto insurance
- Strong communication and interpersonal skills with the ability to work effectively with individuals served, families, and interdisciplinary teams
- Self-motivated, dependable, and committed to providing compassionate, high-quality care
