What are the responsibilities and job description for the Hospice Triage Nurse position at JSSA?
We are currently seeking a compassionate and experienced RN dedicated to providing exceptional care during their most critical moments to serve as a weekday daytime Triage Nurse covering Maryland and Virginia, this role will be office based out of our Maryland office. The Triage Nurse is responsible for identifying and coordinating patient/family needs and providing supportive evidenced based care to patients and families through telephone triage. The ideal candidate will have finely tuned end of life and symptom management assessment skills and be knowledgeable about Hospice care best practices. Homecare Homebase (HCHB) skills and expertise are preferred but not required. The primary responsibility for the position is triaging patient calls via the phone and assisting Team Managers, Physicians and NPs with patient oversight, staff scheduling, and clinical excellence. Experience in hospice and phone triage preferred.
Essential Duties & Responsibilities include (but are not limited to):
- Responds to patient and family calls in a timely manner, accepting direct transfers of daytime triage calls.
- Symptom Management- Assess pain and physical distress and offer clear guidance on comfort measures or medication recommendations.
- Coordinates care of the patient and family with the interdisciplinary care team and communicates calls from patients and family to the team.
- Inform clinicians and Clinical Managers when urgent in-person visits are needed, based on assessment.
- Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
- Under the guidance of the Clinical Directors, this RN is responsible for patients/families including assessing, planning, implementing and evaluating nursing services consistent with the plan of care.
- Communicate with attending physicians and NP and other community resources to report changes and coordinate optimal care and use of resources for the patient/family.
- Maintain regular communication with patients, families, Clinical Director, Team Manager and Team Coordinators regarding patient/family status and unusual or potentially problematic patient/family issues.
- Collaborates with QAPI RN to assist with HOPE, CAHPS, SHP and staff education.
Education/Experience:
- At least 3 years’ experience as a professional Registered Nurse
- Experience in hospice setting preferred.
- Demonstrated expertise in pain management, pain control, symptom management and emotional support.
- Strong telephonic communication, crisis aversion, independent clinical judgement and empathy.
Certificates, Licenses, Registrations:
- Current license as a registered professional nurse in both Maryland and Virginia
- CHPN certification preferred.
- Current Maryland driver’s License and automobile insurance.
Starting salary of $90,000 based on experience
