What are the responsibilities and job description for the Case Manager (Social Services) position at JQ Staffing Services?
We are seeking a compassionate and organized Case Manager supporting residents in a housing and homeless services program.
This role involves hands-on case management, working directly with individuals to help them transition into stable housing while addressing medical, mental health, and social service needs.
Key Responsibilities:
- Complete intake assessments and onboarding for new residents
- Develop and manage individualized service plans with goals and timelines
- Conduct weekly case management meetings with residents
- Assist with housing applications, benefits, and resource coordination
- Support residents with medical, mental health, and social service appointments
- Maintain accurate documentation in systems such as Apricot, HMIS, or similar
- Track progress and provide regular updates and reports
- Participate in program activities, life skills support, and resident engagement
- Provide follow-up support to former residents as needed
Additional Expectations:
- Ability to work a flexible schedule, including occasional evenings or weekends
- Participate in on-call rotation as needed
- Support overall program operations, reporting, and resident care
Qualifications:
- Bachelor’s degree in Social Work, Human Services, or related field
- Experience in case management, social services, or working with vulnerable populations
- Knowledge of mental health, addiction, and recovery support preferred
- Strong organizational, documentation, and communication skills
- Ability to manage multiple cases and respond to crisis situations
