Key Responsibilities
Person‑Centered Support
- Assist individuals with daily living skills: dressing, hygiene, showering, cooking, cleaning, and household tasks.
- Promote independence, self‑advocacy, and individual HCBS rights.
- Support individuals during community outings, social activities, and skill‑building opportunities.
- Transport individuals to medical appointments, community events, and activities.
Health, Safety & Compliance
- Serve as a mandated reporter for any suspected abuse, neglect, or exploitation.
- Support medication administration assistance and document changes in health or behavior.
- Maintain a clean, safe, and organized home environment; report maintenance needs promptly.
- Follow all DBHDS, DMAS, Medicaid Waiver, and Joy Group Home policies and procedures.
Documentation & Communication
- Complete accurate daily notes, incident reports, and required documentation.
- Communicate effectively with the House Manager, team members, and individuals receiving services.
- Attend monthly staff meetings and participate in ongoing training.
Coaching & Skill‑Building
- Teach and model home management skills, personal care routines, and community safety.
- Encourage personal growth, independence, and meaningful participation in daily life.
Professionalism
- Carry out responsibilities in a timely, respectful, and effective manner.
- Uphold confidentiality, professional boundaries, and person‑centered values.
- Perform additional duties as assigned to support smooth home operations.
Minimum Qualifications
- High school diploma or GED required.
- Valid Virginia driver’s license.
- Ability to pass all required background checks.
- Preferred: At least 1 year of experience providing direct care or working in human services.
