Job Summary
The Care Coordinator is responsible for the overall introductions and coordination of care between partnering facilities and Josselyn’s services. This includes intake, assessment, case management, navigating, psychiatric evaluation, medication monitoring, and supported employment. The Care Coordinator plays a vital role in ensuring patients receive comprehensive and timely behavioral health services.
Schedule and Compensation
Monday - Friday, 9 AM - 5 PM
$47,000 - $54,000 annually based on experience
Key Responsibilities
- Coordinate and facilitate communication between partnering facilities and Josselyn’s services.
- Conduct patient intake and assessments to determine care needs.
- Manage cases and provide ongoing support to patients with behavioral health needs.
- Assist patients in navigating psychiatric evaluations and medication monitoring processes.
- Collaborate closely with the psychiatric services team, case managers, Managed Care Organizations (MCOs) such as Illinois Health Practice Alliance (IHPA), and other departments.
- Ensure patients attend scheduled appointments and receive necessary care.
- Work with patients, providers, and community resources to identify and remove barriers to care.
- Maintain accurate and organized documentation and manage time effectively to meet deadlines.
- Provide service coordination for patients with behavioral health needs, including supported employment services.
Qualifications & Skills
- Bachelor's degree in human service field required.
- At least 1 year of relevant experience working with individuals experiencing mental illness.
- Strong organizational skills with attention to detail in paperwork and documentation.
- Excellent time management skills to handle multiple cases and responsibilities efficiently.
- Experience in case management, psychiatric evaluation coordination, and medication monitoring preferred.
- Ability to collaborate effectively with multidisciplinary teams including psychiatric services, case managers, and MCOs.
- Strong communication and interpersonal skills to work with patients, providers, and community resources.
- Knowledge of behavioral health services and supported employment programs is a plus.
