The Scheduler is responsible for coordinating and scheduling patient services to ensure timely, efficient delivery of hospice care. This role works closely with clinical and administrative staff to support patient needs, staff availability, and interdisciplinary care coordination.
Essential Functions:
- Promotes the philosophy, mission, and administrative policies of the Hospice by presenting a positive organizational image
- Schedules, updates, and documents patient schedules accurately and in a timely manner
- Participates in coordinating care with management and the Interdisciplinary Group/Team (IDG)
- Provides effective communication to patients, families, staff members, healthcare professionals, and referral sources
- Responds to Hospice and patient needs in a professional and creative manner
- Demonstrates commitment to professional growth, competence, and Hospice standards
- Performs other duties as assigned
Education:
High school graduate or equivalent required. Graduate of an accredited school of vocational/practical nursing preferred.
Experience:
Minimum of 2 years of experience in healthcare intake or admissions, preferably in hospice.
Skills:
- Proficiency with EMR systems and Microsoft Office.
- Strong organizational and communication skills.
- Ability to manage multiple tasks and prioritize effectively.
Transportation:
Reliable transportation, current auto insurance, and a valid driver\’s license.
Environmental and Working Conditions:
- Works in an office environment.
- Ability to travel locally as needed.
- Ability to work a flexible schedule.
- Some exposure to unpleasant weather.
Physical and Mental Effort:
- Prolonged sitting and computer use required.
