Overview:
The Home Health Branch Director of Clinical Operations / Nursing Supervisor is a registered professional nurse who provides oversight and leadership for all patient/client care services and personnel within the branch. This role ensures the quality, continuity, and appropriateness of care delivered in the home setting. Responsibilities include coordinating care with the interdisciplinary team, patient, family, and referring agency, administering skilled nursing care, and directing clinical staff in accordance with agency policies and procedures.
Essential Functions:
- Supervises and provides direction and leadership to clinical nursing and aide personnel to ensure quality and continuity of services.
- Coordinates and integrates care provided by Agency staff, including contractual providers.
- Ensures communication with all physicians involved in a patient’s/client’s plan of care and other healthcare practitioners.
- Integrates orders from all relevant physicians into the plan of care and ensures approval by the responsible physician.
- Coordinates referrals and services and informs the physician and other personnel of changes in the patient's condition and needs.
- Provides ongoing assessment of home health patients to identify physical, psychosocial, and environmental needs.
- Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s).
- Provides services ordered by the physician, including patient/client and caregiver education and counseling.
- Prepares clinical notes and assures continuity of quality patient care with appropriate documentation.
- Promotes compliance with all fiscal intermediary and/or other third-party payors through education and coaching.
- Monitors systems and identifies problem areas to the Supervising Nurse and Administrator.
- Monitors care to ensure compliance with regulatory agencies and third-party payors.
- Performs on-call responsibilities and provides on-call service to patients and families as assigned.
- Participates in the Agency QAPI program and sponsored in-service training.
- Promotes the philosophy, mission, and administrative policies of the Agency.
- Demonstrates commitment, professional growth, and competency.
- Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
- Ensures the care offered by the Agency, including any branches, is consistently available to all patients/clients.
Job Qualifications:
Education:
Graduate from an accredited diploma, associate, or baccalaureate nursing program.
Licensure:
- Active, unencumbered state license as a registered nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state.
- Current driver’s license.
Experience:
- Two years of experience as a registered nurse in a clinical care setting and one year of home health experience.
- Management experience preferred.
Skills:
- Nursing skills as defined by generally accepted standards of practice.
- Excellent interpersonal and communication skills.
- Demonstrates proven decision-making skills. Proof of current CPR certification.
Environmental and Working Conditions:
- Works in patients’ homes in various conditions and in an office environment.
- Possible exposure to blood, bodily fluids, and infectious diseases.
- Ability to work a flexible schedule and travel locally.

- RN emergency calls may be required.
- Must be available at all times either in person or via telecommunications.
Physical and Mental Effort:
- Prolonged standing and walking required, with ability to lift up to 50 pounds and move patients.
- Requires working under stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition.
- Requires hand-eye coordination and manual dexterity.
