Are you an experienced MDS Coordinator who takes pride in accuracy, compliance, and quality resident care?
We’re looking for a knowledgeable, organized, and compassionate MDS Coordinator to join our team and play a key role in ensuring our residents receive the care they deserve.
WHAT YOU’LL DO
- Coordinate and oversee the MDS assessment process
- Ensure assessments are accurate, timely, and compliant with current CMS requirements
- Collaborate closely with Nursing, Therapy, Social Services, Dietary, and other interdisciplinary team members
- Lead and facilitate care plan development and coordination
- Monitor documentation and identify opportunities to improve clinical outcomes
- Support regulatory compliance and survey readiness
- Help ensure the interdisciplinary team is working together toward the best outcomes for our residents
- Maintain certs/recerts
- Initiate death certificates
WHAT WE’RE LOOKING FOR
- Previous MDS Coordinator experience in a Skilled Nursing Facility
- Strong knowledge of MDS 3.0 / RAI processes
- Understanding of Medicare, Medicaid, and SNF reimbursement requirements
- Excellent organization skills and attention to detail
- Strong communication and leadership skills
- A team player who is committed to resident-centered care
