Job Description

We are seeking a Health Case Manager to support lead exposure prevention and healthy homes initiatives.

The ideal candidate will possess strong organizational skills, critical thinking ability, and experience managing cases within health, environmental health, or community health programs.

Key Responsibilities

• Manage and coordinate assigned cases related to lead exposure prevention and healthy homes programs.
• Communicate effectively with families, physicians, and local health department personnel regarding case progress and requirements.
• Maintain accurate and organized case records using Microsoft Office and Adobe tools.
• Schedule and participate in virtual meetings, webinars, and conference calls with stakeholders.
• Analyze case information and apply critical thinking to resolve complex issues.
• Build and maintain strong relationships with families, healthcare providers, and community partners.
• Adapt to policy or regulatory changes and ensure compliance with program requirements.
• Support documentation and reporting requirements in accordance with health department standards.
  
  

Required Skills

• Proficiency in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint)
• Experience using Adobe (PDF creation and editing)
• Familiarity with webinar and conference call technologies (e.g., Teams, Zoom, WebEx)
• Strong organizational and time management skills
• Ability to solve complex problems and think critically
• Ability to adapt to change in a fast-paced environment
• Strong interpersonal skills and ability to build relationships with stakeholders
