Full Job Description

Job's House is hiring for Individualized Home Support Specialist (IHS) to provide clients with training and resources to develop skills for independent living. Goal is to maximize clients independence in the community. IHS Practitioner will work with clients in various age ranges. Job's House believes in helping the people we serve and our IHS Practitioners will work with our individuals to accomplish their personal and professional goals and aspirations.

Examples of IHS Worker tasks:

Safety in the community; Social Skills; Develop Healthy Relationships; Assist client with Chores (home, laundry); Mail/Paperwork organization; Business needs (Bills, check book); Doctor/Dentist appointments; Community Resources (Recreational/Housing/Employment/Community Support); Meal Preparation and Grocery Shopping to name a few.

Education and Experience:

-High School Diploma/equivalent

-Ability to Pass a Minnesota Department of Human Services Background Study

-Have A Minnesota Driver’s License, Insurance and Reliable Vehicle

-Direct Support 1 year preferred

-IHS/ILS Support 1 year preferred

Duties/Responsibilities:

-Be willing to transport clients

-Able to document through Case Notes, expediently, capturing details of IHS Scheduled visits with clients weekly.

-Transporting to appointments/community activities/grocery store

-Manage a case load of a minimal of 8 hours per week

-Maintain routine schedule

-Assist clients to build skills to navigate transportation options/budgeting/benefits/etc

Skills:

-Be a team player, attend team meetings and offer ideas and resources

-Maintain confidentiality with professionals utilizing releases of information when necessary, adhering to all HIPPA regulations.

-Be organized, ethical and trustworthy

-Knowledge able with apps/timecards

· Complete all training required by State of Minnesota 245D

Job Type: Full-time/Part-Time

Pay: $16-18 per hour

• Monday - Friday 8:00AM - 2:00PM.
• Monday - Friday 2:00PM - 6:00PM.
• Friday - Sunday 8:00AM - 4:00PM
