Responsibilities
- Deliver personalized assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by taking vital signs, observing changes, and reporting findings accurately to healthcare teams
- Administer medications following care plans while adhering to HIPAA regulations to protect client confidentiality
- Assist with meal preparation, ensuring nutritional needs are met and accommodating dietary restrictions
- Operate mobility aids such as Hoyer lifts and assist with transfers safely to prevent injury
- Maintain a clean and safe environment by performing light housekeeping tasks like cleaning and organizing living spaces
- Support clients with dementia or Alzheimer's disease through specialized memory care techniques and compassionate engagement
- Follow detailed care plans developed by healthcare professionals to ensure consistent, quality care
- Document patient observations, care activities, and progress diligently to ensure seamless communication among care teams
Experience
- Prior experience in home health, assisted living, nursing homes, or group homes is highly preferred
- Demonstrated knowledge of patient observation, vital signs monitoring, medication administration, and care plan implementation
- Experience working with individuals with developmental disabilities, autism spectrum disorder, or dementia is advantageous
- Familiarity with HIPAA regulations and maintaining patient confidentiality in all interactions
- Hands-on experience with patient transfer equipment such as Hoyer lifts or other mobility aids
- Strong background in caregiving roles involving long-term care or hospice environments is a plus
- Ability to perform heavy lifting safely when required for resident transfers or equipment handling
- Certifications such as CPR and First Aid are required; additional certifications in dementia or Alzheimer's care are beneficial
- Excellent communication skills to collaborate effectively with clients’ families and healthcare providers
