Jewish Family Services (JFS) is seeking a compassionate, organized, and mission-driven Refugee Health Screening Case Manager to support the health and wellness of newly arrived refugees through timely, culturally responsive medical screening coordination and case management services.

The Refugee Health Screening Case Manager coordinates and facilitates domestic health screenings to ensure communicable diseases of public health importance and other health concerns are identified and addressed through timely screening, referral, follow-up, and care coordination. This position ensures screenings are initiated within 30 days of arrival and completed within required state timelines.

As part of the JFS Health Team, this position works collaboratively across refugee health programs and is expected to support the broader Refugee Health Promotion Program, contribute to integrated service delivery, and assist in filling service gaps as needed to support program continuity and client needs.

This position works closely with partner clinics and healthcare providers, the Office of Global Michigan, and other community partners to support timely reporting and documentation through the Michigan Disease Surveillance System (MDSS), while promoting equitable access to healthcare and successful integration for newly arrived refugee families.

Essential Duties and Responsibilities

  • Coordinate referrals and scheduling for refugee domestic medical screening services in collaboration with refugee resettlement partners.
  • Ensure medical screenings are initiated within 30 days of arrival and completed within 90 days, in compliance with program requirements.
  • Review overseas medical examination records through the Electronic Disease Notification (EDN) system prior to screening.
  • Prioritize screening appointments for clients identified as Significant Medical Condition (SMC) and with Class A or B for medical conditions.
  • Coordinate referrals for immunizations, follow-up care, specialty care, and other services identified through screening.
  • Support clients in navigating healthcare systems and accessing appropriate medical care.
  • Provide client education and training to support independent health management and effective navigation of the healthcare system.
  • Provide client-centered, culturally responsive case management services to newly arrived refugees.
  • Educate clients and resettlement partners about refugee health screening processes and available healthcare resources.
  • Coordinate language-accessible services, including interpretation support, to ensure equitable access to care.
  • Connect clients to internal and external resources that support individual health and social service needs.
  • Promote continuity of care through follow-up and coordination with providers and community partners.
  • Create and maintain Refugee Health Assessment cases in the Michigan Disease Surveillance System (MDSS).
  • Enter demographics and domestic health screening results accurately and in a timely manner.
  • Maintain client files and documentation in accordance with agency, funder, and regulatory requirements.
  • Complete required case notes and documentation in agency data systems including IRS, Unicentric, and related platforms.
  • Ensure timely completion and submission of monthly, quarterly, and annual reports.
  • Support compliance related to Medicaid billing and other reimbursement requirements.
  • Maintain regular communication with refugee resettlement agencies regarding referrals, anticipated arrivals, and service coordination.
  • Participate in quarterly partner meetings to strengthen referral processes and coordination.
  • Serve as an active member of the JFS Health Team and collaborate across programs to support coordinated service delivery.
  • Support Refugee Health Promotion activities and assist with health team service delivery needs, including helping fill program gaps as needed.
  • Collaborate with colleagues to support client care coordination, outreach, health education initiatives, and team-based responses to emerging client needs.
  • Assist with the development of outreach materials, community education, and health-related program initiatives.
Qualifications:
  • Bachelor's degree in social work, Public Health, Human Services, or related field required; master's degree preferred, or equivalent relevant experience.
  • Experience in case management, refugee services, healthcare coordination, public health, or related human services work.
  • Strong written and verbal communication skills.
  • Excellent organizational, documentation, and data management skills.
  • Proficiency with computers and case management/database systems.

Preferred Qualifications

  • Experience working with refugee, immigrant, and culturally diverse communities.
  • Knowledge of refugee health screening processes and public health systems.
  • Experience with MDSS, EDN, or similar electronic client database or systems preferred.
  • Experience coordinating medical referrals and health navigation services.
  • Multilingual skills are strongly preferred.
