PHYSICAL THERAPIST 

 

Jericho Road Community Health Center is actively seeking a Physical Therapist. This position is full-time, within the Physical Therapy Department, working at our 1021 Broadway site in the Gym and Wellness Center.

Responsibilities: 

  • Evaluate, diagnose and treat patients of all ages and backgrounds to help them optimize function, alleviate pain, and enhance quality of life.
  • Provide culturally competent care to a diverse and under served patient population, demonstrating sensitivity to language, cultural practices, and other socioeconomic factors that may impact health and outcomes.
  • Develop treatment plans and continuously update individualized treatment and be able to modify treatments when appropriate to achieve these goals.  
  • Evaluate and record patient progress, modify treatment plans, and try new treatments if required. 
  • Instruct patients and teach them therapeutic exercises, stretches and a range of motions, flexibility, coordination, and balance to improve their condition. 
  • Conduct thorough initial evaluations to determine appropriateness for physical therapy, identify impairments, and recommend appropriate plan of care or referral if outside scope.
  • Participate in preparing physical therapy patients to transition to the gym once their physical therapy treatment is complete. 
  • The therapist will only perform interventions within their scope of practice in which they are competent and/or receive advanced continuing education. 
  • Provide ongoing education and instruction through a written home program to the patient and caregiver.
  • Support clinic growth by participating in outreach efforts including patient follow-up calls, scheduling coordination, community marketing initiatives, and collaboration with referring providers.
  • Maintain an active caseload of approximately 60 patient visits per week while ensuring high standards of care and patient satisfaction.
  • Serve as a clinical instructor and mentor to physical therapy students, supporting their development in evidence-based practice and professional conduct.
  • Perform other duties outside of job description as assigned
  • Maintain patient confidentiality 
  • Communicate with an interdisciplinary team (including primary care providers, community health workers, care managers, administrative staff, etc.) to ensure consistent care to address all aspects of health. 
  • Participate in regular metric reporting as a team and comply with federal and state physical therapy regulations. 
  • Collaborate with HR and follow policy and procedures in on boarding physical therapy students and/or interns ensure compliance of students/interns in all required training 
  • Provide and supervise growth and learning opportunities for physical therapy students and/or interns 
  • Work with credentialing team on all aspects of information as needed 
  • Aid in the opening and closing of the gym facility  
  • Assist in supporting another therapist’s clinical caseload as needed.
  • Must remain current in best practices in industry and licensure 
  • Completes required continuing education requirements as mandated by state licensing.    
  • Chart and track client progress in the EHR (Medent) system

 

Qualifications:                   

  • Doctor of Physical Therapy degree and be licensed to practice in NYS
  • Current CPR/AED and First Aid certification or will obtain through Jericho Road Community Health Center

 

Rate: $68,000-$78,000 salary. Individual compensation is based on various factors unique to each candidate, including skill set, experience, qualifications, and other position related components.
