Job Description
Essential Functions
- Comprehensive PrEP and nPEP navigation services: a range of activities to ensure unfettered access to HIV prevention services.
- Conduct HIV prevention and sexual health risk reduction education and counseling. Engage HIV negative patients in psychoeducation on HIV/AIDS disease acquisition and prevention services, including testing, behavioral and biomedical HIV prevention interventions.
- Schedule/re-schedule telePrEP appointments.
- Provide appointment reminders by patient preferred contact method.
- Support TelePrEP provider and patient in ensuring a smooth and efficient telehealth appointment experience. Liaise between patient and clinical team to support self-testing, STI treatment, and medication refills.
- Link patients to comprehensive primary care when needed.
- Link patients to ancillary services: free condom access, mental/behavioral health supports, and other community services.
- Enhance positive healthcare outcomes and community awareness about HIV/AIDS, impacting community health through education, linkage, referrals and direct service delivery.
- Identify and evaluate models that transform sporadic users of health care into regular and continual users of healthcare; and identify effective methods to support and retain clients in care.
- Respond to PTP referrals and patient messages within 8 business hours.
- Track progress using CareWare, EMR, and MS excel as appropriate; use data to identify needs assessment.
- Provide ongoing adherence and retention services and psychosocial support.
- Assist patients in linkage and navigation of the healthcare system.
- Assist patients with enrolling in insurance and co-pay assistance programs for which they are eligible.
- Contribute to program reporting.
- Comply with reporting requirements including Health Department and funding source.
- Participate in discussions in staff meetings, case consultations, and group supervision.
- Coordinate efforts to recruit and retain patients in HIV prevention services.
- Promptly attend scheduled meetings, in-services and trainings.
- Other projects related to HIV prevention as assigned.

Patient Assessment and Care
- Knowledge of nursing and medical terminology; ability to assess ailments and injuries of patients to triage appropriately
- Knowledge of child and adult education techniques and health literacy barriers
- Ability to elicit learning priorities and needs and educate patients and their designated care partners on the patient's condition, the treatment and medications suggested and given, and the next steps in the patient's recovery.
- Knowledge and application of pharmacology as it relates to primary care practice including medications used to treat patients and immunizations for preventive care.
- Knowledge of pharmacology resources/references for professional and patient use.
