Join James River Home Health and Hospice, where your compassion and expertise help patients thrive at home. We’re looking for a Home Health and Hospice Master of Social Work to provide vital emotional, social, and resource support to patients and families as part of our collaborative care team.
Schedule: Flexible PRN Schedule
Areas of Coverage: Harrisonburg and surrounding areas

A brief overview of Responsibilities:
- Complete psychosocial assessments and participate in patient admissions
- Develop and implement individualized care plans
- Provide short-term counseling, crisis intervention, and discharge planning
- Help patients access community resources and navigate challenges
- Document visits and collaborate with physicians and clinical staff
- Supervise Social Work Assistant monthly
- Participate in team meetings, in-services, and ongoing education
Qualifications:
- Master’s Degree in Social Work (MSW) from an accredited program preferred
- Certification from the Academy of Certified Social Workers (preferred or in progress)
- At least one year of experience in a healthcare setting; two years preferred
- CPR certification and valid driver’s license required
- Criminal background check required
