In this role, you'll provide skilled nursing care to patients in their homes on an intermittent basis.

Brief overview of Responsibilities:
- Support the Registered Nurse in delivering each patient’s plan of care
- Assist with specialized procedures under the RN’s direction and the physician’s orders
- Contribute to planning, implementing, and regularly evaluating patient care plans
- Recognize how social and economic challenges impact patients, and provide emotional and physical support for them and their families
- Help educate patients and families on proper self-care techniques
- Actively participate in team and staff meetings to promote coordinated care
- Document clinical and progress notes after each patient visit
- Monitor and report any symptoms, reactions, or changes in patient condition to the appropriate team member
- Participate in a rotating weekend on-call schedule
- Perform other duties as needed
Qualifications:
- Must be a Licensed Practical Nurse with a current license to practice nursing in the Commonwealth of Virginia
- At least one year of home health experience preferred
- Graduate of an accredited nursing school.
- Must have a valid driver's license and a reliable car available for use during work hours.
