The Hospice Intake Coordinator manages the referral and admission process for patients entering hospice services. This role works closely with patients, families, referral sources, clinical staff, and leadership to ensure a smooth, timely transition into hospice care while delivering exceptional customer service and support.
Schedule?
Schedule: Full-time, in-office, 8:00 AM to 5:00 PM, with participation in a rotating weekend and holiday schedule.

A brief overview of Responsibilities:
- Coordinates hospice referrals and admissions, verifies patient eligibility and insurance coverage, communicates with patients, families, and referral sources, and ensures all required documentation is completed for timely admission.
- Completes daily follow-up on pending referrals and provides admission status updates to leadership.
- Other duties as assigned.
