You will be responsible for providing skilled Occupational Therapy to clients in their homes on an intermittent basis. You will also coordinate with the interdisciplinary team to ensure an optimal care experience for the patient!

Brief overview of Responsibilities:
- Evaluate patients' functional abilities and therapy needs.
- Complete assessments, including OASIS, and monitor progress.
- Assess the home environment and family support to determine care needs.
- Help develop and carry out the patient's care plan with the physician and care team.
- Educate patients and families on therapy exercises, safety, and home care.
- Coordinate visit schedules with the therapy team.
- Complete documentation and progress notes within 24 hours of each visit.
- Perform other duties as assigned.
Qualifications:
- Must be a graduate of an approved Occupational Therapy school
- Must be licensed in Virginia
- Must have at least one year of institutional experience
