PLEASE NOTE : OUR HOSPICE UNIT IS IN NEED OF A PATIENT INFORMATION REPRESENTATIVE. THIS POSITION INVOLVES WORKING IN OUR HOSPICE UNIT WITH TERMINALLY ILL PATIENTS AND THEIR FAMILIES. FULL TIME MONDAY - FRIDAY 10am - 6 : 30pm
Jamaica Hospital Medical Center's Ferrara Family Center for Hospice Care is a 10-bed unit. Our Hospice Center provides comfort care for those with life-limiting illnesses; and our goal is to enhance each person's quality of life medically, spiritually and emotionally. We are currently seeking a full-time Patient Information Representative for immediate opportunity in our in our Hospice Unit.
DUTIES
- Utilizing EPIC software to input, update and retrieve patient data.
- Assisting patients in obtaining services, understanding policies and making health care decisions.
- Assembling charts for patient admissions.
- Coordinating communication between patients, family members, medical staff, administrative staff, or regulatory agencies.
- Interviewing patients or their representatives to identify problems relating to care.
- Maintaining knowledge of community services and resources available to patients.
- Referring patients to appropriate health care services or resources.
- Investigating and directing patient inquiries or complaints to appropriate medical staff members.
