JAMAICA HOSPITAL MEDICAL CENTER, a major health care facility located in Queens, N.Y. is seeking an Ambulatory Care Social Worker for the Population Health/Ambulatory Care department.

Job Responsibilities:
- The Ambulatory Care Social Worker is responsible for providing comprehensive social work services to patients and their families in an outpatient setting.
- This role involves assessing patient needs, developing care plans, coordinating services, and providing support to help patients manage their health conditions and navigate the healthcare system. The social worker will collaborate with a multidisciplinary team to ensure the delivery of high-quality, patient-centered care.
- Provides Care Management services to patients with history of behavioral health diagnoses or concerns and develops interventions to address high or inappropriate utilization
- Completes comprehensive bio-psychosocial assessment, including comprehensive chart reviews for at risk patients, in their home or by phone
- Refers for mental health, and/or substance use disorder services as needed; collaborates with medical and behavioral health providers
