What are the responsibilities and job description for the Social Worker - Mobile Crisis position at Jamaica Hospital Medical Center?
12-hour shift, working from 6:30 PM to 7:00 AM, 3 days a week
The Mobile Crisis Team at Jamaica Hospital Medical Center provides home based mental health intervention, engagement and support services to people experiencing severe behavior crisis. We offer a broad range of services including assessment, crisis intervention, counseling and information and referrals to community-based mental health services. Our Mobile Crisis Team consist of behavioral health professionals including social workers and peer specialist who collaboratively provide short-term management and care.
We are currently seeking Licensed Social Workers for immediate, opportunities.
- As a Licensed Social Worker in our Mobile Crisis Unit, you will provide professional casework services to patients and family members with social and or personal problems related to their illness, hospitalization, and integration into the community.
- Providing supportive therapy, case management, crisis intervention and client advocacy.
- Monitoring, evaluating, and recording client progress with respect to treatment goals.
- Providing psychoeducation to patient and family regarding mental illness.
REQUIREMENTS
- Must be Licensed LMSW, LCSW, or LMHC required. (Open to limited permits)
- Previous inpatient clinical experience in a hospital setting/CPEP, and or in a mobile crisis unit, and or discharge planning strongly preferred.
- Ability to work in an interdisciplinary team environment.
- Ability to multi-task and prioritize in a fast paced, high volume setting.
- Excellent communication, organizational and time management skills. PC Proficiency required.
- Must be available to work rotating weekends and holidays.
SALARY: $33.65/hr. - $47.43/hr. DOE Plus applicable shift differential
Job Type: Full-time
Pay: $38.23 - $55.22 per hour
