What are the responsibilities and job description for the Community Outreach Specialist position at Jacob's Hope?
Jacob's Hope is a dedicated and compassionate 501(c)(3) non-profit organization that provides crucial care to substance-exposed newborns and prenatal support to mothers in need in Arizona. Our mission is to ensure a safe and nurturing environment for both mother and child during this critical phase of their lives. We provide 24-hour care to substance-exposed newborns and support to moms on the path to recovery, for a happy and healthy life.
Pregnant women with SUD face unique emotional and psychological challenges during their pregnancy journey. The fear of their baby being taken away due to their substance use often intensifies their anxiety and prevents them from seeking the necessary support and care they require. It is imperative that we work together to combat not only social stigma, but institutional stigma and create a system that supports and empowers these women to seek the help they need. As a trauma-informed advocate for the well-being of women facing substance use challenges, the Outreach Specialist is essential in community outreach, fostering partnerships, building awareness, and ensuring expectant mothers have access to the services they need.
The Outreach Specialist plays a critical role in the overall success of the organization. The Outreach Specialist works in conjunction as part of a multi-disciplinary team made up of Registered Nurses (RN), Nursing Assistants (NA), Behavioral Health Director, and administrative supports. - Work collaboratively with the Social Service Liaison, Behavioral Health Director, and administrative staff to ensure coordinated outreach and follow-up.
- Demonstrate excellent oral, written, and electronic communication skills.
- Ensure confidentiality of communications and maintain professional relationships with staff, partners, and community members.
