Minimum Requirements

• Bachelor’s, Master’s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Indiana Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience working in skilled nursing or long-term care settings preferred.
• Experience with mobility assessment, therapeutic exercise, gait training, and restorative nursing programs preferred.
• Ability to document patient care in electronic medical records required.
