Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active Illinois license or ability to obtain an Illinois PTA license prior to the assignment start.
• Current BLS (Basic Life Support) certification from American Heart Association or American Red Cross is required or must be obtained prior to start.
• Minimum of 1 year experience working as a PTA in a skilled nursing facility or long-term care environment preferred. This includes experience with restorative programs, transfers, and ADL-focused interventions.
• Experience working with geriatric populations and common SNF diagnoses such as post-surgical care, stroke, and orthopedic conditions preferred.
• Ability to follow POC established by supervising PT and document according to facility and regulatory requirements.
• Must be able to work the specified days and shift hours for this assignment.
