Minimum Requirements

• Doctoral, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program approved by the APTA.
• Current, unrestricted West Virginia physical therapy license required prior to start.
• BLS certification from AHA or ARC may be required.
• Experience with OASIS visits and add-on evaluations required.
• Home health experience preferred.
• Ability to complete documentation in a timely manner and meet productivity expectations (points-based productivity may apply).

Assignment Details

• Facility type: Home Health.
• Shift: Days.
• Shift hours: Monday–Friday, 08:00–16:00.
• Weekly guaranteed hours: 40.
• Coverage area: Kanawha County; local travel between patient visits required. Mileage reimbursement between first and last patient is provided.
• On-call/weekend requirement: None.
• Dress code: Royal blue scrubs.
• Must support community- and state-focused patient populations.
