Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active Illinois PTA license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience working in a skilled nursing facility or long-term care setting preferred.
• Experience treating post-acute, geriatric, or medically complex patients preferred.
• Ability to work independently under PT direction and document per facility and regulatory requirements.
• Client-specific requirement: ability to work 5x8 schedule, supporting days 0830–1630.
