Minimum Requirements
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Current, unencumbered Oregon physical therapy license required to start the assignment.
- Basic Life Support (BLS) certification required from AHA or ARC.
- Minimum 2–3 years of physical therapy experience preferred. This is preferred, not required.
- Experience providing care in home health settings preferred, including home visit evaluation and treatment planning.
- Ability to supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned.
- Familiarity with electronic documentation and basic computer skills including email, electronic health records, and online training platforms.
- Must have reliable transportation, valid driver’s license, and agency-required liability insurance for home visits.
