Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist team in Macomb, MI providing home health services. You will help patients recover from injuries and manage chronic conditions while working with a caseload that supports functional independence and safe discharge to community settings.
Minimum Requirements
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Michigan Physical Therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Experience with Home Health preferred; candidates new to home health will be considered.
- Familiarity with Home Care Home Base EMR preferred.
- Ability to meet productivity expectation of approximately 30 points per week.
- Valid driver’s license and reliable transportation. Mileage reimbursed at IRS standard mileage rate.
Preferred Qualifications
- Prior home health caseload management experience.
- Comfort performing home safety assessments and working independently in patient homes.
- Experience with telehealth or remote documentation workflows.

- Shift days: M-F.
- EMR: Home Care Home Base.
- Interview format: Teams/Video Call.
- Caseload geography: primary hub in Macomb with travel expected within roughly a one-hour radius depending on referral patterns.
- On call: None.
