Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
- Active Michigan Physical Therapy license or ability to obtain required state licensure prior to start.
- BLS certification from AHA or ARC may be required.
- Experience with Home Care Home Base EMR preferred.
- Client requires productivity expectation of approximately 30 points per week.
- Valid driver's license and ability to travel within the coverage area, generally within an hour radius of hub cities listed.
- Willingness to cover the Livonia/Downriver area including Wyandotte, Taylor, New Boston, Lincoln Park, Allen Park, Southgate, Westland, Wayne, Garden City, Inkster, Plymouth, Canton.
Preferred Qualifications
- Previous home health physical therapy experience preferred but candidates new to the setting will be considered.
- Comfort with video/Teams interviewing process.
Assignment Details
- Shift days: M-F.
- EMR: Home Care Home Base.
- On call: None.
- Mileage reimbursement at IRS standard mileage rate.
- Productivity expectation: 30 points per week.
- Clinician may be expected to travel and cover referrals across the Livonia/Downriver hub and surrounding cities, generally within an approximate one-hour radius depending on referrals.
