Minimum Qualifications
- Master’s degree in Physical Therapy required; Bachelor’s or Doctoral degrees acceptable if accredited and APTA-approved.
- Active New Hampshire Physical Therapy license required prior to start.
- BLS certification from AHA or ARC may be required.
- Home health experience preferred; must be able to work in patient homes and community settings.
- Experience with HCHB EMR preferred.
- Ability to meet productivity expectations and weekly hour guarantee of 32–40 hours.
Preferred Qualifications
- Prior home care experience and familiarity with home health documentation workflows.
- Comfort with mileage reimbursement processes for travel between patient visits.
Assignment Details
- Shift hours: 08:00–16:00 (client notes also reference 08:00–16:30 for schedule alignment).
- Weekly hours: 32–40 guaranteed.
- Coverage area: Epping care territory; clinician may travel within assigned coverage area.
- On-call/weekend requirement: no.
- Mileage reimbursement provided for travel between first and last patient visits.
