We are seeking a Qualified Professional Case Manager to provide community-based case management services to at-risk individuals and families. This is a contract-only position focused on assessment, care planning, service coordination, and compliant documentation.
The ideal candidate has experience working with families in the community, can manage a caseload independently of 20-30 families, and understands how to document services accurately within structured programs.
Required Qualifications
Candidates must meet Qualified Professional standards:
- Bachelor’s degree in Social Work, Human Services, Psychology, Public Health, Sociology, or a related field
- Minimum of two (2) years of community-based experience working with at-risk families
- Experience providing case management or care coordination services
- Strong documentation and compliance skills
- Knowledge of community resources and support systems
- Valid driver’s license and reliable transportation
Key ResponsibilitiesClient Services
- Conduct client assessments to identify needs, strengths, and barriers
- Develop and maintain individualized care plans in collaboration with clients
- Provide ongoing case management and follow-up services
- Coordinate referrals to community resources, healthcare providers, and support services
- Support clients in accessing services that promote stability and self-sufficiency
Documentation and Compliance
- Complete all required documentation accurately and on time
- Maintain organized, compliant client records
- Track services, care plan updates, and required forms
- Participate in audits, quality reviews, and supervision as required
Collaboration and Professional Practice
- Communicate regularly with Program Manager and agency leadership
- Participate in required meetings, trainings, and case reviews
- Maintain confidentiality, professional boundaries, and ethical standards
Skills and Competencies
- Strong organization and time management
- Ability to manage a caseload independently

Compensation
