A well established healthcare facility located just outside of Thomasville, Georgia is seeking a full-time, permanent Director of Case Management to join their team!
The Director of Case Management Job Description:
- Full-time, direct hire opportunity
- Mon-Fri schedule, day shift
- Great leadership and administration team!
- Oversee the daily operations and human resource functions of the Case Management department. Offer direction and support to Case Managers and staff, including training on caseload management and the interpretation of regulations, policies, procedures, and organizational goals. Monitor departmental activities to ensure service quality aligns with established standards. Foster relationships with insurance providers, self-insured employers, case management organizations, and healthcare networks.
The Director of Case Management Minimum Qualifications:
- Current CCM® or ACMTM certification is preferred
- Must be qualified to independently complete an assessment within the scope of practice of his/her discipline.
- Must possess bachelor's degree in nursing (BSN) with RN licensure for Nursing
- Open for Social Worker candidates, must possess licensure
- 3 years of hospital-based Case Management experience, including Utilization Review and Discharge Planning experience is required
