We’re looking for a Case Manager at Mercy House, one of our emergency shelter programs in Albany. This role is perfect for someone who thrives on connection, problem-solving, and making a tangible difference in people’s lives.
What You’ll Do
- Build trusting relationships with guests and help them create realistic, empowering goals.
- Assist individuals in accessing housing, healthcare, employment, education, and community resources.
- Partner with DSS, hospitals, and community agencies to ensure seamless coordination of care.
- Support guests through crisis moments with calm, compassion, and professionalism.
- Document services and outcomes accurately to support data-driven success.
- Celebrate each milestone — big or small — as guests move toward stability and independence.
What We’re Looking For
- A people-first mindset and genuine empathy for those facing housing instability.
- Experience in human services, case management, or behavioral health (1 year preferred).
- Knowledge of trauma-informed, harm-reduction, or low-barrier approaches (or a willingness to learn).
- Strong communication, organization, and teamwork skills.
- Valid NYS driver’s license and ability to meet agency insurance standards.
- Associate’s or Bachelor’s in human services or related field preferred (not required).
