Responsibilities
- Evaluate patients’ functional abilities and needs in the home setting
- Develop individualized care plans focused on comfort, safety, and independence
- Provide therapeutic interventions to improve quality of life and maintain function
- Educate patients and caregivers on adaptive techniques, energy conservation, and home safety
- Recommend and train on assistive devices and environmental modifications
- Collaborate closely with the hospice interdisciplinary team (nursing, social work, chaplain, aides, physicians)
- Accurately document patient care and progress
Qualifications
- Licensed Occupational Therapist in Tennessee
- Experience in hospice, home health, geriatrics, or skilled nursing preferred
- Strong clinical knowledge of anatomy and rehabilitation principles
- Compassionate, patient-centered approach
- Excellent communication and teamwork skills
- Ability to work independently in a home-based setting
