Responsibilities

• Evaluate patients’ functional abilities and needs in the home setting
• Develop individualized care plans focused on comfort, safety, and independence
• Provide therapeutic interventions to improve quality of life and maintain function
• Educate patients and caregivers on adaptive techniques, energy conservation, and home safety
• Recommend and train on assistive devices and environmental modifications
• Collaborate closely with the hospice interdisciplinary team (nursing, social work, chaplain, aides, physicians)
• Accurately document patient care and progress
  
  
  

Qualifications

• Licensed Occupational Therapist in Tennessee
• Experience in hospice, home health, geriatrics, or skilled nursing preferred
• Strong clinical knowledge of anatomy and rehabilitation principles
• Compassionate, patient-centered approach
• Excellent communication and teamwork skills
• Ability to work independently in a home-based setting
