As a Home Health Registered Nurse - RN, you will provide home-based care for patients recovering from illness, injury, surgery, or chronic conditions while collaborating with physicians and supervising members of the care team.
Responsibilities of Home Health Registered Nurse - RN:
- Provide home-based nursing care to patients recovering from illness, injury, or chronic conditions
- Conduct patient assessments and monitor patient progress
- Develop and implement individualized care plans
- Educate patients and families on medications, treatment plans, diet, exercise, and home safety
- Collaborate with physicians and interdisciplinary care team members
- Supervise LPNs, CNAs, and aides involved in patient care
- Document patient progress and report changes in condition to physicians
- Coordinate discharge planning and ensure patient goals are met
- Maintain accurate and timely documentation
- Graduate of an accredited nursing program
- Active RN license in the state of Virginia
- Minimum of 2 years of nursing experience with demonstrated competency in hands-on patient care, wound care, and blood draws (hospital experience preferred)
- CPR certification and TB test required
- Knowledge of state and federal home health regulations
- Strong clinical judgment, communication, and critical thinking skills
