What You'll Do
As a Home Health OT, you will deliver personalized therapy that helps patients participate more safely and independently in daily life:
Evaluate functional abilities, routines, safety risks, and barriers within the home
Develop and update individualized therapy plans aligned with physician orders and patient goals
Provide interventions that improve strength, coordination, endurance, cognition, and daily living skills
Teach adaptive techniques for bathing, dressing, grooming, meal preparation, and other activities
Recommend adaptive equipment and environmental modifications that support safety and independence
Educate patients and caregivers on exercises, strategies, equipment, and the plan of care
Track progress, adjust interventions, and maintain timely, accurate documentation
Collaborate with nursing, physical therapy, speech therapy, and other care team members
What You Bring
Graduate of an accredited occupational therapy program with an active Arizona OT license
Minimum of [insert required experience] of occupational therapy experience

Current CPR certification
Valid driver's license, auto insurance, and reliable transportation
