We are currently seeking a Per Diem Occupational Therapist to provide high-quality care. This is a great opportunity for clinicians who value flexibility while still making a real difference in patients’ lives.
What You'll Do:
As a Occupational Therapist, you will provide in-home occupational therapy evaluations and treatment to help patients improve their ability to perform daily activities and maintain independence. You will complete required clinical documentation, develop and implement individualized treatment plans, and collaborate with physicians and the interdisciplinary care team to support each patient’s goals. You will also educate patients and families on therapeutic techniques, safety, and adaptive strategies while contributing to overall care coordination and quality patient outcomes.
Employment Type: Per Diem
Area Coverage: Central or East Valley (travel required to patient homes)
What We Offer:

- FULL IRS Mileage Reimbursement: of $0.76 per mile

Requirements:
- Minimum of 6 months of home health experience as an Occupational Therapist
- Must possess a valid Occupational Therapist license in the State of Arizona.
- Current and valid BLS (CPR) and ACLS certifications
- Negative TB test result and flu shot within the past year
- Up-to-date immunization/vaccination records including COVID-19, Hepatitis B, MMR, TDaP, and Varicella
- Valid Driver’s License with auto insurance
